Health Declaration Form
The Health Declaration Form is a paper document that is distributed to travelers upon arrival in Guam. The form requires travelers to provide information about their health, such as any recent symptoms of illness or contact with individuals who have tested positive for COVID-19. Travelers are also required to indicate if they have been vaccinated against COVID-19 and if so, provide proof of vaccination.
Both Forms are mandatory for all travelers arriving in Guam, irrespective of vaccination status. Non-completion may result in denial of entry or additional screening. Travelers should carefully review the instructions and ensure they have all the necessary documents before arriving in Guam.
How to take a visit permit for a Guam Visa: Options and Procedures
There are two ways to get a permit for travel to Guam by getting a US visa- online or at an embassy or consulate. The online application process is faster and more convenient while applying at an embassy or consulate may take longer but allows for face-to-face interactions with a consular officer.
Guam Visa Waiver Program
Guam has additional Visa Waiver Programs for nationals of certain countries. Nationals of Australia, Brunei, Hong Kong, Japan, Malaysia, Nauru, New Zealand, Papua New Guinea, Singapore, South Korea, Taiwan, and the United Kingdom can travel and stay in Guam for up to 45 days without obtaining a visa or ESTA.
To travel to Guam under the Guam Visa Waiver Program, these nationals need to complete the I-736 Form, which is available online and on paper. They also need to possess a non-refundable return ticket to their home country. However, they are not allowed to travel to other parts of the USA, including the mainland, without a visa or ESTA.
Guam Work Visa
If you are planning to work in Guam, you need to apply for a work visa. The requirements, application process, and processing time for a Guam work visa are similar to those for a US work visa. You need to have a job offer from an employer in Guam and meet other eligibility criteria.
