I am a citizen of Liberia and currently residing in Liberia. I was diagnosed with Polycystic Kidney Disease after several medical examinations and laboratory tests at the government highest referral hospital in Liberia, John F. Kennedy ((JFK). Doctors have since advised me to seek further medical treatment outside Liberia. Kindly see attached medical results and Doctor's summary report.
After several checks and consultations for medical facility outside Liberia, the Istinye University Hospital Bahcesehir in Istanbul, Turkey was identified and contacted for further medical checkup and treatment.
All medical reports from JFK in Liberia was submitted upon request from the hospital in Istanbul. Doctors responsible for my condition reviewed my medical reports and sent me a quotation for medical services in Turkey. Please see attached quotation from the hospital. The hospital asked me to deposit the total amount of $1,550.00 USD (one thousand, five hundred & fifty USD) to enable me receive invitation from the hospital and apply for visa to travel to Turkey.
I have deposited the amount requested. Kindly see attached receipt which the hospital has acknowledged receipt.
In view of the above, I write to apply for medical visa to seek early medication to save my life and return to Liberia. May wife may accompany me to on this.
Do I have to travel to Sierra Leone to for my visa?
