Tourist Visa to Bolivia

I’m a Malaysian, travelling/residing in Malaysia and will be travelling to Peru and Bolivia and understand that I would need an eVisa upon arrival at Bolivia.
As there’s no Bolivia embassy or consulate nearby (in Malaysia), can I do a Visa on Arrival instead with my Malaysian passport?
And can please advise on the immigration form as part of the documentations required (if any) to be presented for Visa on Arrival?
Also, can please advise on the total amount to be paid for Visa on Arrival?
